BOARD OF HEALTH
Meeting Minutes October 22, 2007
Memorial Town Hall

MEMBERS PRESENT:  Martha Waite, Mark Johnson, Mary Ricker

OTHERS PRESENT: Andrea Crete, Agent, Randy Mizereck, Consultant, Debra Sciascia, Recording Secretary

The Board of Health Meeting was called to order at 5:05 PM

SEPTIC PLAN REVIEWS:

585 Manning St, Lot 2 (previously lot 4):  Robert Marino, new construction.  Revised plans submitted and reviewed by the Board per 9/17/07 meeting.  Signed plan given to Robert Marino, approval letter and permit will be mailed separately.
M. Johnson motioned to approve plans.  M Ricker seconded.  All in favor, approved.

Wachusett St., Map 30/Parcel 3:  Spofford Eng. for Ed Oldakowski,  These plans were previously approved by the Board and have expired.  The perc tests are still good.  Revised plans submitted and reviewed by the Board.
M. Johnson motioned to approve plans.  M Ricker seconded.  All in favor, approved.

174 Union St:  James Morin for Andy Brewer, new construction.  Jim Morin explained the system to the Board.
M. Johnson motioned to approve plans.  M Waite seconded.  All in favor, approved.

MEETING MINUTES:
June 11, 2007:  M. Waite motioned to approve with changes.  M. Johnson seconded.  All in favor, approved.
July 23, 2007:  Tabled for changes.
August 20, 2007:  M. Waite motioned to approve with change.  M. Johnson seconded.  M. Ricker abstained.  Vote 2-0, approved with change.
September 17, 2007:  M. Waite motioned to approve.  M. Ricker seconded.  All in favor, approved.

OTHER BUSINESS:
Discussion regarding Holden Sanitation’s recent performance and ‘Disposal Workers Installers Permit’.  Jim Morin is presently operating without a town issued permit.  The Board asked A. Crete to prepare a letter to be sent to Holden Sanitation, with a copy to the homeowner, stating that his permit has expired.  If he wished to renew, he may do so during regular business hours, and the renewal will be taken into consideration by the Board.  He will be prohibited from doing any work in Holden without the permit.
The agreement letters, and sand certification for 20 North St., 450 Sterling Rd., and 786 Princeton St. were never submitted to the Board of Health by Holden Sanitation.
MAHB:  Certification Program information was distributed to the Board for consideration.

Next Meeting:  November 26, 2007 at 5:00 PM

M. Waite motioned to adjourn the Board of Health meeting.  M. Ricker seconded.  All in favor.

Meeting Adjourned at 6:20 PM.
